IVF & fertility insurance coverage in India: a patient's guide
How health insurance treats IVF, IUI and fertility care in India — what is usually excluded, what may be covered, and the questions to ask before you start a cycle.
The short answer
In India, fertility treatment is normally paid for out of pocket. IRDAI's standardisation of health-insurance exclusions permits insurers to exclude “sterility and infertility” treatment, including in-vitro fertilisation, ICSI, IUI, gestational surrogacy and reversal of sterilisation. Most retail indemnity policies use that exclusion as written. Cover, when it exists, comes from a specific add-on, a maternity benefit variant, or an employer's group plan — never assume it from the brochure.

Clinic pricing in India varies widely by city, protocol and drug brand, so a quoted “IVF package price” rarely covers everything. Ask for a written estimate broken into these lines before you commit:
- Consultations and baseline diagnostics (hormone panel, AMH, ultrasound, semen analysis)
- Stimulation drugs — usually the largest variable, priced per ampoule
- Monitoring scans and blood tests during the cycle
- Egg retrieval, anaesthesia and day-care charges
- Lab work: ICSI, blastocyst culture, assisted hatching, PGT-A if advised
- Embryo freezing plus annual storage, and the cost of a later frozen transfer
- What is refunded or carried forward if the cycle is cancelled before retrieval
Your rights at the clinic
The Assisted Reproductive Technology (Regulation) Act, 2021 requires ART clinics and banks to be registered under the National Registry, to obtain informed written consent, and to counsel patients about the cost and the chances of success before treatment begins. Ask to see the clinic's registration and its own success rates for your age band, and treat any guaranteed-baby or refund-scheme offer with caution.
Common questions
- Is IVF covered by health insurance in India?
- Usually not by default. Under IRDAI's standardised exclusions, treatment for infertility and sterility — including assisted reproductive technology such as IVF and IUI — sits in the list of exclusions insurers are permitted to apply, so most indemnity health policies exclude the cycle itself. A small number of insurers offer infertility cover as an add-on, a rider, or inside specific maternity or group corporate plans, so cover depends on your exact policy wording.
- Do employer group policies cover IVF?
- More often than retail policies. Corporate group health plans are negotiated per employer and some include an infertility or ART sub-limit, usually capped in rupees and often with a waiting period and a per-employee lifetime limit. Ask your HR team for the policy wording, not the summary brochure.
- Does the government fund IVF in India?
- There is no national scheme that funds IVF for the general public. Ayushman Bharat PM-JAY does not include assisted reproduction, and CGHS and some state or PSU schemes reimburse only in limited, specified circumstances. Check the scheme's own package list rather than assuming.
- What parts of fertility treatment might still be claimable?
- Even when the IVF cycle is excluded, related hospitalisation can sometimes be claimed — for example, treatment for ovarian hyperstimulation syndrome, or surgery for endometriosis, fibroids or a hydrosalpinx that is medically indicated on its own. Diagnostics and consultations are typically out of pocket unless you hold an OPD benefit.
- What should I ask before starting a cycle?
- Ask the insurer for the exclusion clause in writing, whether an infertility add-on exists and its waiting period, and any sub-limit. Ask the clinic for a written cycle estimate that separates drugs, procedures, lab work, freezing and storage, and confirm what a cancelled cycle costs.
